We make regularly scheduled trips to partner facilities for in person patient care and offer virtual visits to address urgent issues when we are offsite.

Home Team is called to overcome complexities of the American healthcare system to make it easier for patients to access primary care services.
Team Based Care
Working in teams allows us to respond to your needs more quickly and makes in person primary care possible in places where it is not otherwise available. Your team includes you, preferred family members, other medical teams involved in your care, Home Team medical support staff, a dedicated nurse practitioner (NP) or physician assistant (PA) serving the role of Primary Care Provider, and medical doctor (MD) serving the role of Attending Physician.

Getting to know you as human being is the first step to providing compassionate care. Our team structure allows your PCP to budget more time for developing and enjoying personal relationships with you and your family. Your attending physician supports your PCP by advising on challenging medical problems and advocating for you when we need help from doctors and institutions outside of our practice to coordinate your care needs.
Flexible Solutions
We use modern technology and flexible business models to create personalized solutions for patients and their families, regardless of insurance barriers. To avoid the frustration of playing phone tag, we offer several ways to contact our medical team including phone, text messaging, secure web portal, and other secure messaging services. We also offer competitive cash rates when insurance doesn’t cover tests and other services you want.
Supported Staff
Medical teams provide better care when the people on those teams aren’t burned out or stretched too thin. We support our staff with respectful employment arrangements and the best available tools for their job. Working in teams also means we can help each other out and cover for a friend when life events require them to step away from work for a period of time, without causing disruptions in their patient’s care.

We manage all general primary care, including but not limited to problems with blood sugar (Diabetes Mellitus), blood pressure (Hypertension), cholesterol (Hyperlipidemia), Heart disease, Lung disease, Arthritis, common intestinal disorders, and general sick visits.
